Holiday Program Time
Drop Off: 8. 30 am to 9.00 am
Pick Up: Between 3.30 pm and 4 pm *
Sign In/Sign Out Procedure
• Children must be signed in and out of the venue daily by an authorised parent or guardian.
• Sign In and Out Sheets will be provided on the day for the parent or guardian to complete.
• Children will only be allowed to leave the venue with a parent or guardian.
* Please make every endeavour to pick up your child/children on time.
SISA is committed to providing a safe environment for the participating children, which includes ensuring that children are not left alone after any of our programs.
If you are running late to pick your child/children it is essential that you call Jai, Holiday Co-ordinator, on Ph: 0423 544 402.
SISA reserves the right pass on costs if staff are required to stay with children in excess of 15 minutes after the scheduled pick up time.
What to Bring
• Any required medication and a medication plan (eg Asthma Action Plan)
• Lunch & Snacks - please provide a healthy lunch and snacks suitable for your child’s active day
• Water Bottle (clearly labelled with the child’s name)
• Hat (clearly labelled with the child’s name)
• Sunscreen
• Spare clothing and shoes in case of wet weather or accidents
What NOT to Bring
• Food containing Nuts – our programs are ‘Nut Free’ for the safety of all children.
• Valuables, eg jewellery
• Devices – programs are ‘device free’
o If your child needs to make a phone call to you, they can make a request to the Head Teacher on the day and they will allow the child to use a SISA phone.

• SISA takes no responsibility for any devices or valuables brought to the program.
Clothing & Equipment
• Children will need to wear suitable sporting attire and footwear
• Please provide a bag to carry these items
• Ensure all clothing and equipment is clearly labelled with the child’s name
• Long hair needs to be tied back at all times
• No jewellery to be worn
Sun Safety
SISA endeavours to protect all children from the harmful effects of ultraviolet light (UV) radiation from the sun so please ensure that your child:
• Sun Hat – hats must worn at all times as SISA has a ‘No Hat, No Play’ policy for the safety of your children.
• If your child is unable to use Sunscreen for any reason, you must inform the Head Teacher at sign in and ensure your child/children wears suitable clothing to protect them from the sun.
Medical Conditions/Additional Support
If your child has been diagnosed by a doctor as having a medical condition (ie, Asthma, Anaphylaxis, Epilepsy, etc) or requires additional support from us, it is essential that SISA’s Head Teacher is informed when enrolling your child for the program.
Any child who has been diagnosed with Asthma or Anaphylaxis MUST provide an INDIVIDUAL HEALTH MANAGEMENT PLAN completed by a medical practitioner and given to the Head Teacher on arrival.
Infectious Diseases/Illness
• If your child falls ill whilst attending the holiday program you will be notified as soon as possible by a member of staff.
• Any children not well enough to participate in normal daily activities or presents symptoms of illness prior to the start of the day should not attend the day.
Behaviour Policy
Our programs are designed to cater to individuals’ needs in a safe and supportive environment where the children feel empowered and encouraged. Our holiday programs are designed to be enjoyable and your child's safety is our number one priority.
• Staff will encourage good behaviour at all times and discourage any forms of bullying and/or intimidation.
• If a child has been repeatedly spoken to for poor behaviour we will contact the parent/guardian to explain the behaviour and request the parent/guardian pick up the child asap.
Cancellation Policy
• Once a booking has been accepted no cancellation, refunds or credits will be given without a medical certificate being provided within one week of the program for the day/s missed
• A transfer to another day may be permitted if 48 hours’ notice has been given and there is availability on the required day
Parent Waiver and Consent
• By completing this online application form as the parent or guardian I hereby give my full to and approval for my child/children to participate in this program.
• There are risks of injury when participating in every sport and other related activities in the course of the program and I assume these risks on behalf of my child/children.
• In addition to giving my full consent for my child’s participation I do hereby waive, release and hold harmless SISA, it’s officers, teachers and representatives for any injury that may be suffered by my child in the normal course of participation in the designated event and activities incidental thereto, whether the result of negligence or any other cause.
